Summary of position:
This full-time position reports to the Director of Social Work through a Social Work Supervisor or Coordinator and is responsible for providing reentry services to incarcerated individuals receiving mental health treatment.
Responsibilities Include:
Completing clinical evaluations of incarcerated individuals with the purpose of developing appropriate discharge plans.
Liaising with community agency, facilitating communication between the agencies and the department/health care setting and the utilization of available community resources.
Providing appointments and referrals to community mental health and/or substance use treatment as clinically indicated.
Providing referrals for appropriate case management services and supportive housing to eligible individuals.
Providing assistance with benefits, including Medicaid, social security benefits, and public assistance.
Processing discharges by providing individuals leaving jail with appropriate medications, aftercare letters, referrals to community treatment, and Medication Grant Program (MGP) cards when indicated.
Work as part of an interdisciplinary team to provide clinically driven, comprehensive discharge planning to incarcerated individuals with mental illness.
Engage in appropriate communication and collaboration with court players and alternative to incarceration programs when indicated.
Participate in treatment team meetings, supervision, trainings, and other departmental meetings.
Ensure timely completion of all brad H-related tasks for assigned patients.
Ensure compliance with accepted social work practices and principles and operational adherence to government rules and regulations, including compliance with reporting requirements.
Might provide supervision to other social workers, case workers, or clerical staff.
Department Preferences
Excellent interpersonal communication skills and ability to work collaboratively with other disciplines, including DOC staff.
Experience working with individuals with mental illness.
Experience working in with individuals involve in the criminal justice system.
Experience working with community mental health and substance use resources.
Experience working with alternative to incarceration or reentry programs.
Strong clinical skills.
Qualification Requirements:
1. Education
Master’s degree in Social Work with active NY State license in good standing.
Experience
At least two years of post-license experience.
An equivalent and satisfactory combination of education and experience may be considered.
For Assignment to Level III
Valid license as an LCSW issued by the NYSED.
