You Will Be Responsible For:
Initiates patient coordination at the patient’s entry point to the system. Reviews scheduled patients to ensure appropriate educational material, patient assessments/questionnaires, and pre-surgical care coordination occurs according to established timelines and algorithms.
Able to initiate, identify, and develop/modify individual plans of care, as needed, in coordination with appropriate spine program team members. Evaluates patients’ developmental level, identifies physical/emotional needs that may affect treatment, and evaluates support system needs to enhance patient care and outcomes. Completes follow-up assessments and evaluations. Investigates questionable situations. Evaluates effectiveness of interventions and medications and facilitates care plan modifications, as needed. Initiates age-appropriate care, as required. Ensures that patients’ physical, mental, and spiritual needs are met by coordination with appropriate healthcare team members.
Communicates effectively with physicians, healthcare team, patients, and families to ensure coordinated and cohesive execution of evidence based quality patient care; effectively/consistently facilitates with members of the healthcare team to develop treatment plans, increase patient satisfaction, address specific patient concerns and discharge planning; participates in team conferences and provides information relating to therapeutic concerns in a clear, concise manner; reports to program manager problems or concerns involving direct/indirect patient care in a timely/professional manner; establishes/maintains a therapeutic relationship with patients /families.
Develops, coordinates, and documents patient education and care. Serves as a primary ongoing resource for patients and families during their diagnosis and treatment period; assesses patients’ learning style and health literacy to provide education on the appropriate patient level.
Performs follow-up phone calls after discharge and contributes to post-acute care planning to assess effectiveness of program and in helping patients understand and comply with lifestyle changes for future spine injury prevention and /or to clarify any outstanding concerns; assesses patient’s knowledge of discharge instructions including medication adherence; provides/reinforces education as needed regarding medication and dietary regime and diagnosis.
What You Will Need:
Texas Registered Nurse License
BSN with a minimum of 3 years of spine care experience.
Experience with patient/staff education.
Experience with projects or program start-up
2 years of previous leadership experience preferred.
The overall goal of this position is to navigate spine patients within the Texas Health Huguley system to the appropriate care destination and follow the patient throughout the care process. The Spine Nurse Navigator coordinates, delegates, provides, and evaluates care of the spine patients, including planning and delivering timely patient-focused care in partnership with interdisciplinary teams, the patient, and family. The Spine Nurse Navigator facilitates an evidence-based, multidisciplinary, multi-specialty approach to spine care throughout the care continuum at Texas Health Huguley. The Spine Nurse Navigator provides direct clinical service to patients and families including care management and patient education to understand diagnosis, treatment, and follow-up regimen. The patient needs may range from minimal to complex and may occur in the inpatient or ambulatory arena. The Spine Nurse Navigator is responsible for facilitating a patient and physician friendly experience within the Texas Health Huguley Spine Health Program. The Spine Nurse Navigator provide education and mentoring to advance staff knowledge at Texas Health Huguley, actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. The Spine Nurse Navigator assists in planning and conducting community outreach activities, including screenings, as well as assists patients in accessing appropriate resources as needed. The Spine Nurse Navigator facilitates patient support and/or education groups and strives continually for innovative and creative strategies to affect and improve patient quality of life. The Spine Nurse Navigator gathers abstracts, records and reviews data for process and quality improvement, as well as evaluating program goals.
