The Telephone Advice RN is a Registered Nurse who demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. The RN reports directly to and is accountable to the designated Nurse Manager of the Acute Care Clinics (ACC). A Cost-of-Living-Allowance (COLA) of 10.28% will be added to the stated salary. COLA is subject to change at any time. Work Schedule: : Monday thru Saturday 6:00am to 5:30pm; variable to meet the needs of the patient and program needs Financial Disclosure Report: Not required The major duties and responsibilities include, but are not limited to: Uses the nursing process and evidenced-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate, pharmacist, social worker), and larger Team (family/caregiver, internal and community-based services involved providing care to the patient) in developing the patient-driven holistic care plan for life. Provides patient and family symptom-focused health education with a focus on self-management, prevention, and wellness, based on the patient's goals. Demonstrates leadership by serving as an advocate for patients, team plater to colleagues as she/he continues to enhance his/her own professional growth, development and practice. Responsible for being self-directed in the delivery of patient care under minimal supervision by the Nurse Manager.
