Triage Nurse performs care coordination and professional nursing functions to assess the severity of the patient's health concerns using protocols and algorithms in a manner that provides patient dispositions to emergency care, urgent care or clinical provider appointments and triage nurse clinical services within the scope of the licensed practice. The nurse will support providers call center in triaging phone call in patients. Improve the overall health of the communities we serve by enhancing access to nurse-driven advice and guidance.
DUTIES AND RESPONSIBILITIES:
Provides direct support to the call center for incoming patient phone calls with a variety of clinical questions and needs
Facilitates patient care, and documents into the electronic health record
Uses accepted medicals protocols to teach self-care to patient and give medical advice
Determines priority of care based upon physician and psychosocial needs and other factors influencing patient flow
Coordinates with providers and clinical staff to ensure that timely treatment/response is provided to patients
Outreaches to coordinate care for patients with gaps in care including patients recently discharged from hospitals
Manages nurse directed patient telephone calls requesting health care services
Evaluates and assesses patient's health care needs based on patient's signs and symptoms utilizing established triage protocols and guidelines provided by department provider representatives.
Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs
Accurately documents symptoms/complaints, nursing assessments, advice provided and patient/caller response.
Follows policy, procedures, and protocols to ensure consistency and departmental effectiveness as well as improved health care outcomes of patients/callers and their access to appropriate health care
Coordinates and schedules same day appointments on the provider schedules
Manage prescription refills per protocol
Provide patient education: i.e. chronic disease, preventive and family
Participates in Performance Improvement activities as assigned
Assures that pertinent medical information is forward to proper departments as needed for continuity of patient care
Documents in electronic health record (EHR), Triage assessment, plan, implementation, and evaluation, follow-up
Completes other duties as assigned

Current state Registered Nurse Licensure
Bachelor's degree in nursing preferred
Two-year degree required
Triage or in office clinical experience of at least 1 year
