Columbia is the geographic center of South Carolina, with easy access to mountains (two hours west) and nationally recognized beach resorts (two hours east). The Patient Aligned Care Team RN Care Manager integrates and augments health care quality by providing health education, clinical appropriate preventive and therapeutic care, and partnership among patients, personal support persons, and PACT Staff. The RN Care Manager facilitates care coordination and navigation through a complex health care system. It involves working across multiple care settings, to include accessing health care providers and other services in the community to help patients receive the care they need and want without unnecessary duplication of services or avoidable inconvenience. Care coordination involves open communication among health care providers, legally permissible exchange of health care information, and logistical integration of health care encounters. The PACT RN Care Manager participates in population management, a data-driven process for proactively defining a cohort of patient who might benefit from a health care plan or reaching out to individual patients in the cohort to offer the right intervention at the right time, rather than waiting for the patient to self-identify and seek out health care.
