The Social Worker (Program Coordinator) serves as the primary VISN 16 Lead HUD/VASH Program Coordinator, in excess of 200 VISN-wide HUD/VASH Case Managers, toward full implementation of Housing First, oversight of administrative issues, management of data for all VISN 16 HUD-VASH programs, monitoring of performance measures and Action Items. The Program Coordinator works directly with the VISN 16 Homeless Coordinator overseeing an annual budget in excess of 14 million dollars. The Incumbent ensures the work of the HUD-VASH program fits as an integral part of the medical center and that the HUD-VASH staff adheres to all current directives of VHA and meet applicable Joint Commission, CARF and licensure standards. The Incumbent is accountable for full utilization of the 205 vouchers in Alexandria's VAMC catchment area which serves an area of 123 Parishes, and serves four Public Housing Authorities (PHA).Incumbent must not only develop programming that is accountable to the VA but also accountable to Housing and Urban Development Regulations, FR-5213-N-01, which governs the housing subsidy provided by that agency. Duties include, but are not limited to: Exercises full authority and responsibility for providing administrative support to the Homeless Program Manager and Chief of Social Work Service for HUD-VASH issues; Overall planning, organizing, implementing, directing, coordinating, monitoring and evaluating of HUDVASH program; Is the source of instant information for management, employees and patients concerning Alexandria's HUD-VASH Program topics in general, community resource and housing information, in particular and serves as the primary contact to the Public Housing Agencies (PHAs); Manages the daily operation of the program, direct patient care services and supervises social workers assigned to HUD/VASH programs; Responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements for the HUD-VASH program; Ensures that the VISN and organization's strategic plan, mission, vision and values are communicated to the HUD-VASH team and integrated into the HUDVASH team's strategies, goals, objectives, work plans and work products and services; Oversees case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations.
