The RN Community Care Coordinator is responsible for the oversight of non-VA episodes of care. This includes but is not limited to activities necessary to purchase patient care in the community in a manner that successfully integrates the community care with past and future VA care. Major duties and responsibilities include, but are not limited to: Demonstrated clinical competencies in such areas as age-specific care, cultural assessment, and health education. Communicate with tact and respect. Interact collegially with Leadership, Supervisors, peers/colleagues, internal and external customers. Demonstrate the ability to effectively function autonomously, lead clinically integrated teams and/or serve as member to interdisciplinary team(s) Coordinate the process of care with the other team members to maximize efficiency. Document all patient care activities in a timely manner (per facility policy). Identify areas for continuing education and participate in in-service training (including staying current with all required education). Participate in developing and implementing the performance improvement plan (including quality monitors). Serve as a role model in the effective operation of an interdisciplinary team. Utilize VHA and community resources as appropriate. Work Schedule: Monday - Friday, 8:00 am - 4:30 pm Financial Disclosure Report: Not required
