The Nurse practices as an RN in the COVID Vaccine Clinic, administering COVID Vaccines to veterans within VA ECHCS. The RN is responsible for collaborating with services internal and external to the VA in order to facilitate care transition in order to effectively meet the patient's needs. The RN provides patient, family, and caregiver health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The Nurse demonstrates a sustained high level of clinical practice and leadership to improve patient care and excellent interpersonal relationship skills in interpreting and communicating service and hospital goals, objectives, policies, and procedures to staff, patients, and the health care team. The RN Care Manager's practice is modeled on the standards established within the VHA model for Patient Aligned Care Teams. The incumbent practices as an RN in the outpatient clinic setting. The incumbent promotes evidenced based practice, patient driven care. The incumbent identifies, educates and coaches to support self-management, prevention, and wellness, based on the patient's goals. The incumbent manages the care of walk-ins. Uses motivational interviewing techniques to guide patients to establish goals. The incumbent practices within their nursing scope of practice and functional level, demonstrating and promoting the mission of the VA and ICARE values. Performs other duties as assigned. Work Schedule: 40 hours a week. The tour of duty is generally 0730-1600. The clinic runs 7 days a week and applicant may be required to work weekends to meet the mission of ECHCS. Negotiation of tour will be completed prior to start of employment. Financial Disclosure Report: Not required
