This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Duties include but are not limited to: The incumbent independently provides clinical psychosocial and case management services at an advanced practice level to eligible veterans and their family members in the VA Supporting Housing (VASH) Program to meet biological, psychological, social, and environmental needs. The incumbent supports the Health Care for Homeless Veterans (HCHV)/Community Care Programs, including outreach activities. The incumbent is required to travel to patient homes and outreach sites via a government vehicle. The incumbent is responsible for program oversight, monitoring, and administration, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations. The incumbent works closely with the VA medical center's homeless programs, residential and community care programs, and domiciliary programs to ensure coordination between these entities is facilitated and veterans' direct care, referral, and follow-up are contiguous and based on a continuum of services. The incumbent independently provides clinical psychosocial and case management services at an advanced practice level for eligible veterans and their family members in the program. The incumbent supports the local Medical Center through the assignment of special projects and committees. The incumbent provides support and mentoring to social work interns and newly hired social workers. The incumbent supports the HCHV/ Community Care program and Mental Health Service Line by service on task forces and special assignments. The incumbent must be knowledgeable of the principles, concepts, theories, and methodology of clinical social work practice. The incumbent must be knowledgeable of the administrative mechanisms needed for successful program development, management, and evaluation. The incumbent must know the normal developmental theory, including the aging process and basic gerontology concepts, knowledge of adult and geriatric psychology, and knowledge of the physically disabled and the frail elderly's special needs. The incumbent must be knowledgeable of VHA programs and community housing programs that serve the homeless. The incumbent must be skilled in using computer applications related to data collection and interpretation, program evaluation, and clinical services. The incumbent must have the ability to communicate both orally and in writing with people from diverse backgrounds. The incumbent must have the ability to function independently, meets deadlines, and function under pressure. Work Schedule: Mon-Fri 8:00 AM to 4:30 PM. The selectee may be required to work or travel between locations. The selectee may also be required to work various shifts on a permanent or temporary basis, based on patient care needs.
