Outpatient Specialty Social Workers are assigned to one or more specialty clinics and/or specialty programs in departments such as Emergency Medicine, Hospital and Specialty Medicine, Neurology, and/or Operative Care Service include primary and secondary health care services. Outpatient Specialty Social Workers work closely with other members of the assigned specialty care interdisciplinary treatment teams to which they are assigned. Major duties include but are not limited to: Psychosocial treatment and intervention, specialized case management and care coordination, discharge and/or after-care planning, health education and advanced health care planning, supportive counseling and caregiver support, and administrative and professional duties. Provides medical and behavioral social work services for Veterans who present in the ED. Services often involve care coordination and consultation with medical, nursing, and social work staff in other care areas to which Veterans are referred, consulted, and/or also treated and/or co-managed. Works closely with other members of the emergency department and will work closely with members of associated inpatient and outpatient interdisciplinary treatment teams. The incumbent assesses and provides appropriate social work emergency care treatment options and interventions, and ED discharge and placement options. The incumbent provides assessment, evaluation, treatment, and reporting for cases of suspected or reported abuse, neglect, or financial exploitation which present or are identified in the ED. The incumbent participates with members of the inpatient acute medical, surgical, and/or critical care teams and disciplines for discharge planning during evening, weekend, and/or holidays. Serves as program coordinator and is administratively responsible for the Medical Foster Home Program. Coordinator manages the daily operation of the program, and provides direct patient care services in the program area. Responsibilities include the daily operation of the program, developing policies and procedures for program operation and preparing reports and statistics for facility, VISN, and national use. Manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. In conjunction with the Service Chief and Ambulatory Care Manager, plans organizes and directs administrative, operational, patient and personnel activities for the service. The incumbent is capable of practicing at the advanced level and has specialized knowledge of practice typically related to a particular diagnosis or population. The incumbent should have a least 2 years in an area of specialized social work practice and licensed or hold certification in a state at the independent practice level.
