The Whole Health Coach (WHC) is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching, promotion, and education regarding Whole Health Coaching to Veterans and staff. This position is integral to supporting Whole Health programming and Veterans in navigating the Whole Health Journey, specifically through the Pathway, Wellbeing, and Whole Health clinical care elements of the Whole Health System, as well as assisting in the collection of whole health qualitative data from Veterans and creation of reports analyzing effectiveness of health coaching. The Whole Health Coach provides care to Veterans seeking self-directed, lasting changes aligned with their values. They provide interventions that promote health and wellness and enhance well-being, improve health-related outcomes, reduce likelihood of inpatient admission, and improve quality of life. During their work, Health Coaches display unconditional positive regard for their Veterans and a belief in their capacity for change, and honor that each client is an expert on his or her life while ensuring that all interactions are respectful and non-judgmental. Health Coaches work with Veterans on identifying the Veterans' motivation for change, determining goals for their work together, developing treatment plans to reach these goals, implementing those treatment plans, and assessing progress in order to adjust plans as necessary. Health Coaches provide information on the relationship between lifestyle and health outcomes, exploring behaviors that increase the risk of developing or worsening chronic disease and those that reduce the likelihood of chronic illness. Health Coaches collaborate with Veterans and Veterans' primary care and specialty care treatment providers to develop goals designed to improve health outcomes, reduce risk from chronic illness, and lessen the likelihood of emergency, inpatient, and long-term care admissions. Duties include, but are not limited to the following. The Licensed Vocational Nurse (Whole Health Coach): Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in an outpatient setting utilizing face-to-face, Video Connect (VVC), or telephone clinics as appropriate. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other groups as appropriate. Provides follow-up care through telephone clinics or secure messaging between face-to face appointments. Actively guides Veterans in the exploration of core elements of personal health planning Conducts outreach activities to inform Veterans and family members of available programs. Provides educational opportunities for staff-including lunch and learns, grand rounds, referrals to these services. Meets with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans. Collaborates with internal and external resources for supporting the Veteran's Whole Health journey by developing resource lists for well-being support within the facility and in the local community. Develops and conducts onsite educational series for enrolled Veterans, family members and VA employees to explain health coaching, including its uses and benefits. Develops and distributes locally approved health education materials to identified enrolled Veterans and family members. Serves on Healthcare System, VISN, and/or National VA committees or task forces specifically relating to Whole Health or Health Coaching. Represents Primary Care or Patient Centered Care/Whole Health on decision-making committees within the Healthcare System. Participates in leadership roles on local, state and/or national professional organizations specific to health coaching. Represents the Health Care System at professional (VA/Community) programs and/or events. Participates in committees or work groups addressing health coaching, and routinely participates in national VHA conference calls and audio conferences on these and other issues such as motivational interviewing (MI) and other behavioral interventions to promote health.
