The PACT RN is a skilled nurse who is able to work proficiently with some level of supervision to optimize patient care, patient safety and operational efficiency for patients. The focus of the individual's performance encompasses a combination of clinical, administrative, education and research. The PACT RN must demonstrate performance that impacts more than one individual and patient care assignment. This PACT RN uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal, and community -based services involved in providing care to the patient) in developing the patient-driven care plan for life. The PACT RN is a member of the Teamlet team. H/She is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The PACT RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's personalized health plan/goals. The PACT RN demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development, and practice. He/she possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting to facilitate the effective care of patients. Within the framework of the nursing process and through highly developed clinical expertise, the RN is able to assess, monitor and treat patient responses to life-threatening health problems. He/she is responsible for collaboration with administrative and clinical staff in actively managing and planning for actual and potential needs of patient population groups Work Schedule: Varies by location; Monday-Friday, 7:30 am - 4:00 pm.; M-F 10 hour shifts, 1 day off; Overtime, including evenings, Saturdays and Federal Holidays, may be required.
