Work Hours/Shift

Full time Exempt, 40 hours per week

You Will Be Responsible For

Patient/support partner(s) and staff education: Promotes knowledge of surgical procedures and effects for patient, support partner(s), and care providers

Performance Criteria
o Meets with patient/support partner(s) prior to scheduled surgery to provide education on surgical procedure, post-operative expectations and care needs.
o Collaborates with direct care staff to provide ongoing education for surgical procedures, evidenced-based practice and/or research developments.
o Initiates discharge planning and provides education on post-discharge care resources and supplies.
o Follows patients throughout surgical experience, performs daily rounding to ensure continued communication with patient, staff and provider
o Collaborates with existing community organizations on educational initiatives.
 

Quality Management: Educates and collaborates with multidisciplinary team to plan and participate in quality initiatives and research projects. 

Performance Criteria
o Reviews patient information for pre-existing medical conditions and risk-factors. 
o Serves as customer service advocate for patient and support partner(s).
o Consistently collaborates with physicians and multidisciplinary team to establish and oversee plan of care. 
o Reviews physician and nurse documentation to ensure quality measures are achieved.

Clinical Services: Serves as primary point of entry for patients requiring navigation and coordination of care. Collaborates with patient to identify potential barriers to achieving post-surgical health goals.

Performance Criteria
o Reviews patient information to follow up on diagnostic tests and obtains results of previous tests as needed. 
o Monitors patient’s progress through post-operative phase to ensure patient reaches post-operative goals.
o Collaborates with nursing care department(s) to confirm continuity of care

Community Outreach: Contributes to community programs and education pertaining to specific surgical services

Performance Criteria
o Maintains a working knowledge of the availability of community resources and financial services for providing clinical care and reducing barriers to care.
o Fosters supportive professional partnerships with community physicians.
o Participates in select community events to promote specific surgical programs at AdventHealth Hendersonville.
o Collaborates with community resources commonly used for post-operative care needs. 

Program Development: Coordinates with multidisciplinary team and providers 

Performance Criteria
o Develops and monitors program improvement projects based on data analysis
o Tracks and analyzes performance for core measures, patient outcomes and patient experience. 
o Develops and implements education programs and materials for patients and staff
o Establishes and maintains collegial relationships with physicians, multidisciplinary team and leadership to ensure positive outcomes and continuous program improvement.
o Leads Joint Center operations team meetings.

What You Will Need

Bachelor of Science Degree in Nursing (BSN) or Master of Science in Nursing (MSN) required at time of hire or enrollment in MSN program within 1 year of hire

At least 3 years of recent nursing experience in acute care nursing required

At least 2 years of recent nursing experience in orthopedic care

Current BLS certification required, ACLS preferred

Professional certification in specialty area by recognized national nursing organization within 1 year of hire required

Current RN licensure in the state of North Carolina required

Job Summary

The orthopedic Nurse Navigator is a specialized hhealth care professional who guides specific surgical patients through the healthcare sylstem during and surrounding the time of surgery.  The Navigator will focus on these five areas:  patient support partner(s) and healthcare provider education, qualify management, clinical services, community outreach and program development.
