ChristianaCare is looking for a Community Health Worker to the join the team serving residents of Wilmington in multiple community based locations. In response to COVID-19 and needs within our community of Wilmington, we have identified several community access points to deliver care directly on-site at the centers. Currently, we provide COVID-19 testing along with a virtual provider assessment at both the Kingswood Community Center and Latin American Community Center to deliver care in a different way. This medical team reports on-site to these locations in a non-traditional setting, and connects all patients to testing/services (medical and community) as needed. We are looking for a team who is willing to be flexible as we continue to expand our services throughout the communities we serve. Join our team on this exciting new journey! Community Health Workers promote access to services, provide health education and assist community members in navigating the health care and social services systems. This Community Health Worker will specifically work within the Community Care Practices - COVID19 testing stations. PRINCIPAL DUTIES AND RESPONSIBILITIES:
EDUCATION AND EXPERIENCE REQUIREMENTS:
SPECIAL REQUIREMENTS: Access to personal vehicle. Must be bilingual. Ability to work flexible schedule, including some nights/weekends as needed. Knowledge of New Castle County. PHYSICAL DEMANDS: Light to moderate lifting (20-40 pounds) of boxes of resource materials for distribution at meetings, training sessions, etc. WORKING CONDITIONS: Working at Community Care Practice sites (in community) and office plus moderate to extensive off-site travel within New Castle County, during normal working hours, with occasional and weekend hours required.
