BARGAINING UNIT POSITION
The Cardiac Rehabilitation Registered Nurse will provide comprehensive, multidisciplinary services for individuals with cardiovascular diseases using a disease management model of care. As per the United States Public Health definition these services will include, "medical evaluation, prescribed exercise, cardiac risk factor modification, education and counseling." As per the American Association of Cardiovascular and Pulmonary Rehabilitation Core Competencies statement, the cardiac rehabilitation professional's services are, "associated with the case management function which includes the coordination of the patient's multidisciplinary treatment plan," (AACVPR, 1994).
Provides inpatient and outpatient cardiac rehabilitation services to optimize individualized patient goals for the secondary prevention of coronary and other vascular diseases, patients with congestive heart failure and patients at high risk for developing coronary heart disease as established by a risk profile.
Collaborates with all members of the cardiac rehabilitation team and other providers to maximize the patient's individualized plan of care, achievement of patient goals and cost effectiveness of services provided.
Works under the general direction of the Coordinator and Medical Director of the Cardiac Rehabilitation Program.
Performs duties to support the goals and objectives of Geisinger in its efforts to provide quality patient care and patient satisfaction.
Directly accountable to the Team/Clinic Coordinator of Cardiology/Cardiac Rehab.
COMPETENCIES AND SKILLS:
Demonstrates the application of the current standards of care to provide comprehensive, individualized inpatient and outpatient care to patients with cardiovascular disease.
Demonstrates excellent assessment skills, ability to work both independently and collaboratively and be accountable for practice.
Demonstrates excellent problem solving skills, time management skills and be able to effectively communicate verbally and in writing.
Demonstrates the ability able to perform duties that result in the highest level of patient care and patient satisfaction. Able to role model behaviors and lifestyle habits that promote cardiovascular health.
EDUCATION AND/OR EXPERIENCE:
Registered Nurse currently licensed in the state of Pennsylvania required. Graduation from an approved school of nursing required. Bachelor's degree in Nursing preferred.
Current knowledge of nursing management of patient's with cardiovascular disease required. Current knowledge of exercise physiology preferred.
Minimum one year caring for patients with cardiovascular diseases. Minimum one additional year experience in a critical care nursing area (ICU/CCU/ED) required. Cardiac rehabilitation experience preferred. American Heart Association BLS and ACLS certification required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Must be able to work flexible hours with various start times. Required standing, sitting and performance of stretching exercises both standing and on the floor.
