As a member of the Interdisciplinary Team, the Clinical Social Worker promotes and maintains the mental and social health of the enrolled participants through assessment, treatment, teaching and counseling. Provides basic casework and consultation. Facilitates communication between participants, their family and the IDT. Facilitates the participant council to create dialogue among participants, caregivers and the staff. Responsible for the implementation of social work care plan and coordination of social work and other services.
Reports to the Center Manager, LIFE Geisinger, Geisinger Community Health Services.
COMPETENCIES AND SKILLS:
Demonstrates strong interpersonal, oral and written communication skills.
Demonstrates ability to be a self-starter with strong organizational and time management skills.
Demonstrates ability to be detailed oriented, extremely well organized and ability to plan and problem solve.
EDUCATION AND/OR EXPERIENCE:
Master's degree in Social Work required, valid Pennsylvania licensure to practice preferred.
Minimum one year experience and interest in working with the elderly required. Community Health experience preferred.
Pennsylvania Driver's License and ability to use vehicle for work required.
Computer literacy, with specific experience in Microsoft applications and Windows operating platform, required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in an office environment. Commonwealth of Pennsylvania driver's license and car available for work. This position is considered light in nature as defined by the U.S. Department of Labor. This position is classified as exposure category III concerning occupational exposure to the hepatitis B virus (HBV) and the human immunodeficiency (AIDS).
