This is a temporary position that will last through February 2020.
The region’s leading network of healthcare providers, Inspira Health is seeking an experienced professional to join the team. Working on a multidisciplinary health care team in a primary care setting, the Population Care Coordinator (PCC) will focus on coaching and coordination of care for high-risk and at-risk patient populations. The PCC will work alongside providers and office staff engaged with the Population Health department, focusing on identifying needs while developing processes for managing patient populations. The PCC plays a key role in managing patients with complex needs and chronic conditions in order to improve the overall health of local populations.
Associate’s degree; Bachelor’s preferred
New Jersey RN licensure; Certified Case Management (CCM) certification preferred
5 to 8 years of direct patient care experience
Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare or managed health plan; care management role preferred
Familiarity with community and social services resources to assist patients with social needs
