Be part of our growing Value Based Care team! We are currently seeking a experienced Care Management Supervisor that is an MSW or LCSW to workat our location in Wilmington, DE. This position is Monday- Friday.
Primary Function -
The Care Management Supervisor is a Social Worker and is responsible for providing oversight to the VBSO Non-Clinical care management team. This team includes Community Health Workers (CHW) and Non-clinical and Social work (MSW) care coordinators in Delaware and Pennsylvania. This position will report to the Manager of Care Management and be responsible for supervision and daily management of the non-clinical care management team in conjunction with the clinical team. The supervisor will assist in developing a centralized non-clinical care coordination team and a community based CHW team.
Essential Functions –
Works collaboratively with VBSO medical management teams and leadership to achieve and sustain best practices and staffing targets to support care management activities across the continuum for the populations served.
Collaborates with manager of care management to implement and standardize a care coordination program.
Assists with maintenance and measurement of the VBSO contract performance standards
Participates in interviewing and training of VBSO medical management staff
Coordinates and provides oversight of VBSO medical management new associates to ensure they meet department expectations within 90 assessment.
Serves as a resource to the VBSO medical management team to ensure knowledge and understanding of VBSO contracts.
Analyzes current processes and data and makes recommendations for process improvements
Collaborates with community providers to develop access for patients and improve patient outcomes.
Provides direct supervision for social work licensure.
Clinical Social Work License preferred or will need to obtain within 2 years of hire
Supervisory Experience Preferred
Previous Care or Case management experience Preferred
Pediatric Experience Preferred
