Job Title: Nurse Practitioner Physician Assistant Psychiatric Geisinger At Home
Location: Western Region – Centre, Mifflin Counties - within patient's homes
Work Schedule: Days/Evenings/Rotation/On Call
Work Type: Part Time
Geisinger at Home is a portfolio of services designed to provide integrated clinical care that manages patients with multi-morbid medical conditions primarily within their homes by delivering:
• Comprehensive medical care in the home
• Community based palliative care and coordinated handoffs to hospice
• Acute care by mobile paramedics
The program is designed to deliver a spectrum of clinical services that help high-cost, complex patients with care at home to avoid tertiary settings when possible.
The Nurse Practitioner (NP) or Physician Assistant (PA) works in conjunction with the Home Care Physician, RN Case Manager, Social worker, and Pharmacist to provide a team approach in caring for the most complex patients. The NP/PA will provide direct hands on quality care in the patient’s home. The NP/PA will develop care plans and interventions to meet the patients care needs, while connecting with the primary care provider.

Demonstrates strong professional level of knowledge and comprehensive clinical assessment skills in the complex patient population and chronic disease management.
Demonstrates strong diagnostic and treatment skills.
Demonstrates strong decision-making skills with ability to function in a collaborative care model.
Demonstrates the ability to effectively work with physicians, professional and non-professional staff and contracted vendors.
Demonstrates the ability to be a self-starter with ability to handle multiple projects at one time.
Demonstrates excellent organizational skills, well organized, efficient and able to work under pressure.
Demonstrates customer service experience and skills.
Demonstrates strong written and verbal communication skills which will meet standards of practice and regulatory requirements.
Must have certification of completion from an approved program for Nurse Practitioners/ Physician Assistants and have or be eligible for current license in the Commonwealth of Pennsylvania. Must have BLS and have or be eligible for ACLS.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Ability and willingness to travel
Ability and willingness to take call after hours and participate in the weekend call rotation
Work is typically performed in the patient’s home
